The California Mental Health Planning Council provides an opportunity at each regular meeting for persons to present comments on mental health issues to the Planning Council.

Persons wishing to present comments are requested to comply with the following guidelines:

• Persons making a presentation shall complete the remainder of this form and return it to the Planning Council office, 1600 9th Street, Room 420, Sacramento, CA 95814, (916) 651-3839, or fax it to (916) 651-3922 at least 3 days before the meeting.
  
  
• The presentation should not exceed five (5) minutes.
  
  
• Please submit a copy of your comments in writing, if at all possible, at the Planning Council meeting.
  
  
• Pursuant to state law, the Planning Council will not act on matters presented during the Public Comment period other than to receive the comments and, if deemed necessary by the Council, to refer the subject matter to the appropriate committee for follow-up or to schedule the matter on a subsequent Council agenda.
  
  
• If you need reasonable accommodations for access to our meeting, please call the Planning Council office three (3) weeks prior to the meeting.
